Conquest Home Care Plus, Inc. is a CHAP accredited Medicare/Medicaid Certified Home Health Agency serving clients in Broward County. We provide private assistance and support services for individuals in their homes, hospitals, nursing homes, or assisted living facilities. We make every effort to assign service providers according to the needs of our clients. Prior to any assignment, all service providers must undergo a Level 2 background screening which is a fingerprinting check of state and federal criminal history information conducted through the Florida Department of Law Enforcement (FDLE) and Federal Bureau of Investigation (FBI). Employment and personal reference checks are also conducted. All our service providers are certified/licensed, insured and bonded. When you contact us, a representative from the agency will visit with you and/or your family member to conduct an interview. This will assist us in determining the right health care package for you. You participate in the development of your care plan. We accept assignment of benefits through most long-term care insurances.
